Rhinoplasty

Rhinoplasty – plastic, aesthetic surgery procedure for correcting and reconstructing the shape of nose, aimed at creating psychological comfort for the patient. Unlike other facial plastic surgeries like blepharoplasty or face-lifting for instance, rhinoplasty is not only an anti-aging plastic surgery. It is one of the most difficult and creative plastic surgeries, as it is the nose that largely determines the characteric facial features. In order to avoid an artificial shape of nose even the smallest changes should be made by an esthete-perfectionist plastic surgeon having essential professional skills and experience.

With the help of rhinoplasty it is possible to increase or reduce the size of nose, its shape can be changed, innate or acquired defects and deformations can be eradicated, and problems with breathing can be solved. Rhinoplasty can be performed on patients who are dissatisfied with the shape or size of their nose, while septoplasty (nasal septum surgery) is made in case the patient has problems with breathing. Rhinoplasty often includes removal of the nose hump, size reduction of the nose or its tip, correction of the curviture and thinning of the nose, which improves nose proportions in relation with other facial traits creating a harmonic look.

In order to determine the scope of a plastic surgery, rhinoplasty can be a partial or a full surgery of the nose. In both cases it can be secondary, which means that the surgery was already performed beforehand but the results were not satisfactory (revision rhinoplasty). Aesthetic rhinoplasty can be performed using endonasal closed approach or external open approach according to the choice of the surgeon.

Partial rhinoplasty - external nose job is aimed at increasing the imagery expression of the nose shape, while maintaining individuality and the facial features. An operation which can only soften or stress some of the nose contours, without making significant changes to it.

Partial rhinoplasty is limited to treating only certain sections of the external nose, in some cases without affecting the bone tissue, for example light oval of nose wall in the profile perspective, nose tip correction surgery for eliminating deformations – big, angular tip with hilly defects, excessively upturned or lowered tip, nostril asymmetry, columella, nose wings, etc.

ЧPartial rhinoplasty is also used in order to eliminate changes in the shape of a nose tip, nostrils and uneven nose skin associated with aging. The so called “anti-aging rhinoplasty” takes place when the patients do not aim at making significant changes to their shape of the nose. It is often used after face-lifting, blepharoplasty or other anti-aging facial procedures.

Ринопластика, Частичная, Полная, Хуцидзе Зураб Вахтангович, Зураб ХуцидзеFull Rhinoplasty - aimed at making notable and significant changes to the shape of nose and facial features. While looking at the face profile it can be clearly seen that, due to its shape, the nose is excessively protruded from the face – long straight nose or a long nose with a hump. In the frontal projection – it is the width of the nose wall, large tip, aquiline nose, etc. Full rhinoplasty means changing the shape of a nose by affecting all composing tissues (bone, cartilage, soft) and structural parts of external nose: length reduction, removal of nose hump, narrowing the width of dorsum (osteotomy), correction of tip and nose wings.

Revision (Secondary) Rhinoplasty - aims to correct deformations resulted from the first, unsuccessful surgery. This surgery is complex because the first rhinoplasty was made by the specialist, who used his own plan of solving the problem and the nasal tissues have already passed the post-operational recovery process. The consultation on rhinoplasty is not limited to examining the shape of external nose, and as a rule the whole nose cavity is inspected.

Restoration of nasal breathing  – surgery performed in the nose cavity.

Surgery for the restoration of nasal breathing aims at removing blockage in the nasal air passages (left, right), the reason for which might be the deformation of septum or hypertrophy of nasal turbinate (concha). During the restoration of nasal breathing the shape of the nose is not changed, since it is performed in the nasal cavity without affecting the external nose.

Септоластика, Восстановление дыхания носа, Хуцидзе Зураб Вахтангович, Зураб ХуцидзеSeptoplasty or rhinoseptoplasty is a surgery for the restoration of deviated septum in the left or right nasal passages. In terms of location the deformation may be in the beginning, middle or the back part of the septum, which defines the scope of the surgery. Often conchapasty is required together with the septoplasty, aimed at reduction of hypertrophy of nasal turbinate. After septoplasty the shape of external nose is unaffected, since it is carried out internally, in the direction of the airflow to the nasopharynx.

Conchoplasty -reduction of the volume of hypertrophied turbinates, which is blocking the nasal passage fully or partially. Turbinate can be increased in the left, right, or both nasal passages simultaneously. Even while having an even septum, the enlarged turbinate can block nasal breathing. Conchoplasty is often made together with the rhinoplasty or septoplasty. During consultation the nose cavity is observed and in most cases the CT is required.

Post-traumatic rhinoplasty with the restoration of nasal breathing

This is a synthesis of two surgeries: rhinoplasty and restoration of nasal breathing (operation of both external and internal nose together) In most of the cases it is carried out after a trauma (post-traumatic rhinoplasty) or in case of innate deformation of the nasal septum or due to the acquired deformation of different origins.

Ринопластика, Восстановление носового дыхания, Хуцидзе Зураб Вахтангович, Зураб ХуцидзеPost-traumatic rhinoplasty with the restoration of nasal breathing - includes all plastic and aesthetic surgeries on external nose enumerated above, and a surgery in the nose cavity for the removal of nasal septum and turbinate deformations.

Post-traumatic rhinoplasty is carried out also in cases when external nose is crooked, but the nasal breathing is not totally damaged, that is when the patient is not complaining about the difficulties of nasal breathing.

This type of rhinoplasty is needed in order to fix the post-traumatic crookedness of external nose, after reduction of the size or making changes to the shape of external nose, not to worsen the nasal breathing.

Consultation about rhinoplasty is not limited to examination of anatomic structure of external shape of a nose, as a rule the nasal cavity is always inspected and in most cases the CT of nasal cavity and paranasal sinuses is required.

Surgical Planning

During consultation the plastic surgeon will carefully examine anatomical structure of the nose, will determine the possible changes that can be done to the shape of it, taking into consideration your facial features, will determine the level of healing, the skin condition, aging factors, etc. Throughout the planning process of the surgery, computer (digital) nose model is created. In order to determine the reasons behind difficulties in nasal breathing CT of nasal cavity and paranasal sinuses might be required.

In case of revision (secondary) rhinoplasty it is advised to have pre-operational photos in order to correctly determine the causes of the problem.

Prices and brief information about the surgery

Rhinoplasty – plastic, aesthetic nose surgery

Primary rhinoplasty - full, partial rhinoplasty – without restoration of nasal breathing, price from… Rub. Including general anesthesia, one night at the hospital, subsequent bandages. Blood test, electrocardiogram and fluorography are not included in the cost.

The total cost of the operation is determined by the degree of difficulty, depending on the scope of intervention and the shape of the nose. Excluding the time anesthesiologist needs to perform necessary manipulations, the operation lasts for 1-1.5 hours. Tampons are removed on the second day after the surgery, a plaster cast is worn for 5-7 days with periodic replacement, swelling and bruising under the eyes fade away after 6-12 days.

Revision (Secondary) rhinoplasty (performed earlier by a different surgeon) – without restoration of nasal breathing, price from …Rub., Including general anesthesia, one night at the hospital, subsequent bandages. Blood test, electrocardiogram and fluorography are not included in the cost.

The total cost of the operation is determined by the degree of difficulty, depending on the scope of intervention and the shape of the nose.
